Imaging Studies

The diagnostic workup for suspected mesothelioma begins with imaging. A standard chest X-ray may show pleural effusion (fluid around the lung) or pleural thickening but is insufficient for diagnosis. High-resolution CT of the chest and abdomen is the primary imaging tool, revealing pleural masses, pleural thickening patterns, mediastinal lymph node enlargement, and disease extent. PET-CT scanning using fluorodeoxyglucose (FDG) identifies metabolically active tumor tissue, helps distinguish malignant from benign pleural disease, and detects distant metastases for staging purposes. MRI is sometimes used to assess chest wall or diaphragm invasion before surgical planning. For peritoneal mesothelioma, CT of the abdomen and pelvis with contrast is essential.

Biopsy and Pathological Diagnosis

Tissue biopsy is mandatory for definitive mesothelioma diagnosis. For pleural disease, thoracoscopic (VATS) pleural biopsy provides the largest tissue samples and highest diagnostic yield. CT-guided needle biopsy is a less invasive alternative but may yield insufficient tissue. The biopsy specimen must be reviewed by a pathologist experienced with mesothelioma — ideally at a dedicated mesothelioma center — because mesothelioma cells (particularly the epithelioid subtype) can closely resemble adenocarcinoma cells and may be misdiagnosed. Immunohistochemical staining for mesothelioma markers (calretinin, WT-1, D2-40, CK5/6) and exclusion markers (CEA, TTF-1, BerEP4) is essential for accurate diagnosis. The three histological subtypes — epithelioid (best prognosis), biphasic (intermediate), and sarcomatoid (worst prognosis) — influence treatment planning and legal compensation levels.

Mesothelioma is a cancer of the mesothelium — the thin membrane lining the lungs (pleura), abdomen (peritoneum), or heart (pericardium). It is distinct from lung cancer, which originates in the lung tissue itself. Mesothelioma is caused almost exclusively by asbestos exposure and does not arise from smoking. Lung cancer, while also caused by asbestos in some cases, is far more commonly caused by tobacco smoking. Both are compensable through asbestos trust funds and litigation, but mesothelioma claims typically produce higher compensation because the disease is more directly and singularly attributable to asbestos.
