Risks with Exactech joint replacement devices with defective packaging

Design:
Agency review
Participants:
Agency safety review; no enrolled patients.
Finding:
Oxidation of plastic parts packed in bags missing an oxygen barrier layer can lead to faster wear, cracking and revision surgery.
Main limitation
It does not predict which implant will fail, and FDA does not recommend removing implants that work well.

Delamination and oxidation in compression-molded polyethylene

Design:
Other study design
Participants:
519 Optetrak inserts removed from patients between 2000 and 2019.
Finding:
64 of 519 inserts (12%) had peeling plastic. The authors linked the nonconforming bags to early oxidation and peeling.
Main limitation
Only removed parts were studied, so it cannot give a failure rate for all patients.

The short answer

Which Exactech knee parts were recalled?
Plastic parts in Optetrak, Optetrak Logic and Truliant knees were recalled. The bags they came in were missing an extra oxygen barrier layer. The recall began in August 2021. [14]
Which Exactech hip parts were recalled?
Some hip liners were recalled in 2021, and more in 2022. A liner is the plastic piece inside the hip socket. A smaller XLE liner recall began in late 2024. [15]
Which Exactech ankle parts were recalled?
Plastic liners in the Vantage ankle were part of the 2021 recall. They had the same bag problem as the knee parts. Some Vantage parts were also in a November 2023 recall over a vacuum-seal problem. [16]
How do I check whether my part was recalled?
Ask your surgeon or hospital for the name and serial number of each part. Exactech has an online check. Type in every digit, even zeros. [4]

Which Exactech knee parts were recalled?

Plastic parts in Optetrak, Optetrak Logic and Truliant knees were recalled. The bags they came in were missing an extra oxygen barrier layer. The recall began in August 2021.

FDA lists the knee recall as Class II. Exactech started it on August 30, 2021, and FDA posted it on October 4, 2021. On February 7, 2022, Exactech widened it to all of these knee parts packed in the faulty bags, whatever the label date.

FDA says about 80% of the knee and ankle parts made since 2004 came in faulty bags. Parts made from 2004 to August 2021 may have been put in patients until February 7, 2022. Later recalls covered some knee parts with a vacuum-seal problem, starting in November 2023, and some Optetrak kneecap parts in faulty bags in 2024.

Which Exactech hip parts were recalled?

Some hip liners were recalled in 2021, and more in 2022. A liner is the plastic piece inside the hip socket. A smaller XLE liner recall began in late 2024.

In June 2021, Exactech corrected some Connexion GXL liners used in AcuMatch, MCS and Novation hips because of a risk of early wear. In August 2022, it widened the hip recall to liners packed in faulty bags. FDA classed both as Class II.

The recall that began December 31, 2024 covers certain lots of AcuMatch, Novation and Alteon XLE liners made from 2018 to August 2021. Exactech’s 2022 letter had said XLE liners were not part of the earlier recall. The newer notice is lot by lot, so check your exact part.

Some AcuMatch, Novation and Alteon XLE hip parts were also in the November 2023 vacuum-seal recall. In 2024, some AcuMatch bipolar liners sold in Puerto Rico were recalled too.

Which Exactech ankle parts were recalled?

Plastic liners in the Vantage ankle were part of the 2021 recall. They had the same bag problem as the knee parts. Some Vantage parts were also in a November 2023 recall over a vacuum-seal problem.

FDA’s record covers Vantage fixed-bearing and mobile-bearing liners. Like the knee parts, those in faulty bags were made from 2004 to August 2021. Ankle patients can use the same serial number check as knee patients.

How do I check whether my part was recalled?

Ask your surgeon or hospital for the name and serial number of each part. Exactech has an online check. Type in every digit, even zeros.

Exactech’s checker says it covers its pre-2025 product recalls. If you have a question about a later notice, ask your surgeon’s office to match your part to it.

Exactech’s recall page lists legal@exac.com for questions about its recalls. For implants placed before October 31, 2025, it lists a different address for claim questions, which our bankruptcy claims page explains.

Does a recalled part need to come out?

Not always. FDA does not recommend surgery to remove an Exactech implant that works well and causes no symptoms. Ask your doctor about checkups.

FDA explains that a recalled implant does not always have to be removed. Doctors weigh the risk of taking it out against the risk of leaving it in. Exactech’s 2022 hip letter also advised against removing hips that work well and cause no pain.

Does a recall mean I have a claim?

No. A recall is a safety step, not a court ruling or a payment. A lawyer can review what happened with your implant and your records.

The federal court cases about these parts name Optetrak, Optetrak Logic and Truliant knees, some hip liners and Vantage ankles. Those cases have been paused since November 2024. Under the bankruptcy plan, injury claims against Exactech now go to a trust, which our bankruptcy claims page explains.
