Start with the implant note or device card if you have it. Ask the hospital for details. Do not guess from a picture or company name.
The Bard order discusses several models and excludes Simon Nitinol cases from that particular order. This is why precise product identity matters. A note saying only filter does not answer every product question.
Keep scans and reports of later care together. Put the doctor’s diagnosis apart from your concern about the cause.
For a legal conversation, note the dates and providers for suspected harm, and preserve earlier clot records too. The Bard order describes disputed allegations; that court summary does not supply your missing clinical finding.
Ask the provider how to get covered health records. There are exceptions to access rights. A helper does not have a right to see every record.
As a preparation step, list the facilities involved in placement, follow-up and retrieval attempts. Mark which reports are missing. Ask what permission or representative documents the facility requires rather than sharing another person's records on an assumption.
No. A filter study describes a group, not your implant or injury. It cannot prove that one device caused one person’s harm.
PRESERVE reports 1429 consenting participants and 1421 implants, with benefits and adverse events under its stated goals. It was not randomized and did not determine a lifetime risk for every model. Keep the study apart from your individual reports.
They explain what happened and what was found. FDA describes a tool to help remove some filters after other ways fail. It is not a plan for everyone.
Keep a report even if no filter was removed at that visit. Ask the clinician to explain the finding and next care decision. Do not arrange a procedure simply to create evidence for a claim.
List the implant and care dates. Mark details you are not sure of. Keep case papers and agreements for a lawyer to check the time limit.
A personal deadline depends on the applicable claim and law. Ask what additional records are needed and why before signing broad authorizations. This preparation list does not promise that a firm will accept the case.
